Patrick Wilson Pharm.D., BCACP, CDCES, Health Coach

Patrick’s passion is to create positive outcomes for those who suffer from chronic medical conditions. His belief is that if health care providers can hold the interest of patients, while keeping the patient at the center of care, patient engagement will endure.

Patrick has been a Pharmacist for 27 years and during that time he has practiced in a few key areas that have been impactful on his vision for promoting optimal health.

As an independent pharmacist in San Diego, he established lasting rapport with thousands of patients. The population included patients with kidney failure requiring dialysis; heart failure, Solid Organ Transplant (Kidney, Pancreas, Heart) recipients; patients with Diabetes; and patients who required pain, high blood pressure, HIV / ART, and high cholesterol management. For each of these populations, Patrick developed a team-based approach with Doctors, Nurse Practitioners, Physician Assistants, and nursing staff to follow lab values, make recommendations, and coach the patients. This collaboration allowed him to sharpen his clinical skills, and earn certification in: Ambulatory Care, Diabetes Self Management and Education, and Health Coaching.

Patients were referred to the Health Coaching program after evaluation by the prescribing team. Patients who were given clearance by their doctor started the health coach pharmacist guided concepts of basic mobility, flexibility, stretching and simple exercises, combined with lifestyle and behavior modification. Adapted from the 2015 Dietary Guidelines for Americans, “Eat this not that” became a fun way of advising pharmacy customers on simple ways to incorporate healthy food choices. Patrick used motivational interviewing techniques to help foster an environment of change and to bring about a sense of community among the patients. Many patients went on to enjoy new exercise habits of bicycling, hiking, kayaking, walking, swimming and running. The program clearly demonstrated lower a1c, cholesterol and triglycerides. Patients with nicotine addiction benefited from a compassionate approach. Many dozens of smokers who selected a quit date, adhered to medication therapy, and maintained the change in behavior went on to become nicotine free over 7 years.
