Why Choose Us
The pharmacist coach can communicate to your prescribing team recommendations about drug therapy; we can help you stay adherent to medications and lifestyle changes.
Taking your meds, as prescribed, and keeping your appointments with your Doctor, are two of the most important things you can do for your chronic health condition!

Help Delay Or Prevent Chronic Health Conditions
We are committed to building community and providing interventions that will lessen the impact of or help delay or prevent chronic health conditions across communities far and wide and bring tremendous clinical value to members and health care providers.

Your Health, Our Goal
We are a pharmacist-led company, driven by outcomes and patient experience. We will keep you at the center of care and we will fully understand your situation.
You'll be the driver of decisions as we partner with your Health Care Providers to give you the direction you need to manage a wide range of health conditions effectively.
Local Businesses Support Restorative Hobbies
We believe Hobbies play key roles in coping and resiliency, and re-creating community. A hobby can be an additional therapeutic approach to a health condition. Local, small business providers of hobby supplies and gear provide the perfect venue to build community.
As such, for every member who joins PSHC, PSHC will donate appropriate types of Restorative Hobbies from locally owned Hobby Shops to Dialysis Centers.

Social Determinants of Health
Discover How Society Influences Your Health
Social Determinants of Health are things about our society that impact your health. For example, if you experienced embarrassment as a child or adolescent due to body image issues or other issues out of your control, you may have less healthy behaviors as a coping mechanism.
Smoking, ridicule, loneliness, poverty, and genetics are just a few of the many facets of society that determine our health. In fact, there are many social parts that impact our health.
By talking about these issues and actively participating in identifying them, we can get a better handle on how we cope and become more resilient.
